◆ 내년 개원 30주년 인하대병원 도서지역 공공의료 선도

내년 개원 30주년을 맞는 인하대병원이 인천 도서지역 공공의료의 새 모델을 구축하고 있다. 인하대병원은 25일 1996년 개원 이래 필수 및 공공의료 중심의 상급종합병원 역할을 수행해 왔다고 밝혔다. 인천 옹진군을 중심으로 한 도서지역은 고령 인구 비중이 높고 의료 인프라가 절대적으로 부족하다. 백령도, 대청도, 연평도 등 유인도 24개로 구성된 옹진군은 보건지소를 제외하면 병의원이 백령도에 단 1곳뿐이다. 인하대병원은 이러한 현실을 공공 필수의료기관의 과제로 인식하고 개원 2주년이던 1998년 6월 옹진군 백령면 주민 701명을 대상으로 첫 의료지원 봉사활동을 실시했다. 정형외과 등 5개 진료과, 19명의 의료진이 참여한 이 활동은 인하대병원 도서지역 의료지원의 출발점이 됐다.

◆ 27년간 의료진 698명 참여…주민 9400명 진료

인하대병원은 1998년 영흥면과 덕적면으로 활동 범위를 넓혔다. 2010년대 중반까지 백령면·연평면·덕적면·자월면 등 옹진군 20개 섬과 강화군 교동면·삼산면까지 아우르며 내과·외과·소아과 등 전문의 중심의 순회진료 체계를 지속적으로 운영했다. 지난 27년간 옹진군 등 도서지역 무료진료에 참여한 의료진은 698명, 진료 받은 섬 주민은 약 9400명에 이른다. 2015년 이후에는 일회성 지원에서 벗어나 공공보건 시스템 구축으로 방향을 전환했다. 도서지역 고령 인구의 가장 시급한 의료 수요가 만성 근골격계 통증 관리라는 점에 주목해 재활의학과를 중심으로 한 맞춤형 진료를 강화했다. 2014년부터 전문의 직통 핫라인을 시행하고 2015년에는 백령병원 등 옹진군 도서지역에 원격협진시스템을 구축했다. 2018년부터는 옹진군 주민을 대상으로 심뇌혈관질환 예방 교육과 심폐소생술 교육을 실시하며 치료 중심에서 예방 중심의 공공보건 활동으로 영역을 확장했다. 인하대병원은 2019년 옹진군수 표창을 받으며 민관협력 공공의료 모델로 평가받았다.

◆ 1섬 1주치 병원 지정…스마트 원격협진 시스템 구축

인하대병원은 2015년 인천시 섬프로젝트 일환으로 옹진군 도서지역 민관 협력을 위한 업무협약을 맺었다. 2019년 말 코로나19 팬데믹으로 도서지역 의료지원을 일시 중단했으나 2022년 12월 대청면 주민 100여 명을 대상으로 통증 치료, 영양 수액, 치과 진료 등을 무료로 진행하며 재개했다. 2023년 인천시·옹진군과 '1섬 1주치 병원' 무료진료 업무협약을 체결하고 대청면과 백령면 2개 면을 담당하는 의료기관으로 지정됐다. 같은 해 2월에는 백령병원과 연계한 '스마트 원격 화상협진 시스템'을 구축했다. 인하대병원 통합관제센터의 중환자 전문 의료진이 실시간으로 환자 상태를 관찰하고 진단에 참여함으로써 도서지역 중증·응급환자 진료의 한계를 획기적으로 개선했다. 올해 초에는 옹진군과 의료비 감면 지원 내용을 확대·현행화하는 협약을 맺어 비급여 진료비 할인, 건강검진비 감면, 수술 및 시술비 감면 등 다양한 혜택을 제공하고 있다. 인하대병원 관계자는 "앞으로도 인천 도서지역 주민들의 생명과 건강을 지키는 핵심 공공의료 파트너로서 섬 의료의 새로운 표준을 계속해서 만들어갈 계획"이라고 말했다.

Inha University Hospital, celebrating its 30th anniversary next year, is establishing a new model of public healthcare for Incheon's island regions. Inha University Hospital announced on the 25th that it has served as a tertiary hospital focused on essential and public healthcare since its opening in 1996. Island regions centered on Ongjin-gun, Incheon have a high proportion of elderly population and absolutely insufficient medical infrastructure. Ongjin-gun, consisting of 24 inhabited islands including Baengnyeong-do, Daecheong-do, and Yeonpyeong-do, has only one clinic on Baengnyeong-do excluding public health centers. Recognizing this reality as a task for essential public medical institutions, Inha University Hospital conducted its first medical support volunteer activity for 701 residents of Baengnyeong-myeon, Ongjin-gun in June 1998, two years after opening. This activity, involving 5 departments including orthopedics and 19 medical staff, became the starting point for Inha University Hospital's island medical support.

◆ 698 Medical Staff Participated Over 27 Years…9,400 Residents Treated

Inha University Hospital expanded its activities to Yeongheung-myeon and Deokjeok-myeon in 1998. Until the mid-2010s, it continuously operated a specialist-centered mobile clinic system covering 20 islands in Ongjin-gun including Baengnyeong-myeon, Yeonpyeong-myeon, Deokjeok-myeon, and Jawol-myeon, as well as Gyodong-myeon and Samsan-myeon in Ganghwa-gun. Over the past 27 years, 698 medical staff participated in free medical care for island regions including Ongjin-gun, and approximately 9,400 island residents received treatment. After 2015, the hospital shifted from one-time support to building a public health system. Noting that the most urgent medical need for the elderly island population was chronic musculoskeletal pain management, the hospital strengthened customized treatment centered on rehabilitation medicine. The hospital implemented a direct hotline to specialists starting in 2014 and established a remote consultation system in island regions of Ongjin-gun including Baengnyeong Hospital in 2015. From 2018, the hospital expanded into prevention-centered public health activities by providing cardiovascular disease prevention education and CPR training for Ongjin-gun residents. Inha University Hospital received a commendation from the Ongjin County Governor in 2019 and was recognized as a public-private partnership public healthcare model.

◆ Designated as '1 Island 1 Primary Hospital'…Establishes Smart Remote Video Consultation System

Inha University Hospital signed a business agreement for public-private cooperation in Ongjin-gun island regions as part of Incheon City's Island Project in 2015. Medical support for island regions was temporarily suspended due to the COVID-19 pandemic in late 2019, but resumed in December 2022 with free pain treatment, nutritional IV, and dental care for approximately 100 residents of Daecheong-myeon. In 2023, the hospital signed a '1 Island 1 Primary Hospital' free medical care agreement with Incheon City and Ongjin-gun and was designated as the medical institution responsible for two areas: Daecheong-myeon and Baengnyeong-myeon. In February of the same year, a 'Smart Remote Video Consultation System' was established in connection with Baengnyeong Hospital. Critical care specialists from Inha University Hospital's Integrated Control Center observe patient conditions in real-time and participate in diagnosis, dramatically improving the limitations of treating severe and emergency patients in island regions. Earlier this year, the hospital signed an agreement with Ongjin-gun to expand and update medical expense reduction support, providing various benefits including non-covered medical expense discounts, health checkup fee reductions, and surgery and procedure fee reductions. An Inha University Hospital official said, "We plan to continue creating new standards for island medicine as a core public healthcare partner protecting the lives and health of Incheon island residents."
